Would you like to provide support and inspiration to clients by promoting positive goal-directed behaviors and values and help them successfully transition back into their communities?
Job Responsibilities / Essential Functions                                                                         
  • Have reliable access to personal transportation to meet with client caseload one–on-one at home, school, and/or job locations at nights and on weekends for curfew checks through face-to-face and phone contacts 2 -3 times weekly
  • Conducts drug screens
  • Transport clients in company vehicle and supervising at community service activities
  • Report on clients assigned to caseload with regards to school and/or job attendance
  • Work as part of a team and maintain communication with the program manager, staff, probation officers, and community agencies
  • Participate in staff meetings, training, and in-service meetings
  • Document activities and incidents as required by procedures and complete all required documentation in the electronic health record.
  • May be required to telework (work from an alternative location) as an infection control or prevention strategy, based on timely information from public health authorities about pre-influenza, pandemics, public health emergencies or other similar conditions and provide telehealth services in a responsible manner that provides for client confidentiality requirements in communications as outlined in the telehealth policy
Required Skills
  • Minimum 21 years of age
  • Valid Alabama driver’s license and acceptable driving record with proof of personal vehicle insurance
  • Effective oral and written communication skills (read, write, speak)
  • Effective organizational and time management skills

The Tracker is a non-exempt (hourly), 24 hours per week position.: Monday-Thursday, 5:30 p.m.-9 p.m., every other Friday and Sunday, and occasionally a.m. on Saturdays. Hours may vary.
Must Have Acceptable Background & Drug Screen.  EOE
